Writing Your Own Last Chapter

This session begins with a DVD that focuses on the lives of local families as they take an active role in their own end-of-life planning. This film follows patients, their families and their caregivers as it explores end-of-life issues from several perspectives. The screening will be followed by a discussion of the resources available through www.closure.com that will help you and your families start the conversation about final wishes. You will learn how to put your wishes into writing and complete legal documents that will protect your choices.
